
Habilitation Specialist 1

- Provide support and guidance to individuals with disabilities in their daily activities.
- Develop and implement individualized support plans based on each individual's needs and goals.
- Assist individuals with daily living skills, including personal hygiene, meal preparation, and household tasks.
- Collaborate with other team members, including therapists, social workers, and medical professionals, to ensure holistic care for individuals.
- Maintain accurate and timely documentation of services provided and progress made by individuals.
- Monitor and report any changes in behavior or physical health of individuals.
- Create a safe and inclusive environment for individuals to thrive in.
- Advocate for the rights and needs of individuals with disabilities.
- Participate in training and professional development opportunities to enhance skills and knowledge.
- Foster positive relationships with individuals, their families, and caregivers.
- Follow all state and agency regulations and policies in providing services.
- Maintain confidentiality of all personal information of individuals.
- Communicate effectively with individuals, team members, and external stakeholders.
- Participate in team meetings and case conferences to discuss progress and develop strategies for improvement.
- Continuously assess and evaluate individual progress and adjust support plans accordingly.
- Represent the agency and promote its mission and values in all interactions.
- Complete any other duties assigned by supervisor.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Diverse Team.
Bachelor's Degree In A Related Field, Such As Psychology, Social Work, Or Special Education.
At Least 1 Year Of Experience Working With Individuals With Developmental Disabilities Or Mental Health Disorders.
Knowledge Of State And Federal Regulations Related To Habilitation Services And Supports.
Experience Developing And Implementing Individualized Habilitation Plans And Monitoring Progress Towards Goals.
