- Provide exceptional customer service to residents and assist them in accessing support services.
- Develop and maintain relationships with community organizations and service providers to ensure a comprehensive network of resources.
- Conduct thorough assessments of individuals' needs and connect them with appropriate services, such as healthcare, housing, and employment assistance.
- Coordinate and monitor the delivery of services to ensure timely and effective support for residents.
- Maintain accurate records and documentation of all client interactions and services provided.
- Collaborate with other team members to identify gaps in services and develop strategies to address them.
- Stay up-to-date on local, state, and federal policies and regulations related to support services.
- Communicate effectively with clients, colleagues, and service providers to ensure a smooth and efficient process.
- Provide guidance and support to clients in navigating complex systems and advocating for their needs.
- Continuously evaluate and improve upon service delivery processes to better serve the community.
- Participate in training and development opportunities to enhance skills and knowledge related to support services.
- Maintain confidentiality and professionalism in all interactions with clients and colleagues.
- Represent the State of Illinois and its commitment to improving the lives of its residents in a positive and respectful manner.
Strong Communication And Interpersonal Skills To Effectively Work With Diverse Populations.
Bachelor's Degree In Social Work, Public Administration, Or A Related Field.
Minimum Of 2 Years Of Experience In Social Services, Case Management, Or Community Outreach.
Knowledge Of State And Federal Regulations Related To Social Services And Benefits.
Ability To Multi-Task, Prioritize, And Problem Solve In A Fast-Paced And Dynamic Environment.
