  1. Provide exceptional customer service to residents and assist them in accessing support services.
  2. Develop and maintain relationships with community organizations and service providers to ensure a comprehensive network of resources.
  3. Conduct thorough assessments of individuals' needs and connect them with appropriate services, such as healthcare, housing, and employment assistance.
  4. Coordinate and monitor the delivery of services to ensure timely and effective support for residents.
  5. Maintain accurate records and documentation of all client interactions and services provided.
  6. Collaborate with other team members to identify gaps in services and develop strategies to address them.
  7. Stay up-to-date on local, state, and federal policies and regulations related to support services.
  8. Communicate effectively with clients, colleagues, and service providers to ensure a smooth and efficient process.
  9. Provide guidance and support to clients in navigating complex systems and advocating for their needs.
  10. Continuously evaluate and improve upon service delivery processes to better serve the community.
  11. Participate in training and development opportunities to enhance skills and knowledge related to support services.
  12. Maintain confidentiality and professionalism in all interactions with clients and colleagues.
  13. Represent the State of Illinois and its commitment to improving the lives of its residents in a positive and respectful manner.

Job Qualifications
  • Strong Communication And Interpersonal Skills To Effectively Work With Diverse Populations.
