
REHABILITATION CASE COORDINATOR I (UMP)

- Collaborate with other professionals, such as therapists and healthcare providers, to ensure comprehensive care for clients.
- Connect clients with community resources and support services to aid in their personal and professional growth.
- Maintain accurate and up-to-date client records and documentation.
- Communicate effectively with clients, their families, and other team members to ensure a coordinated and person-centered approach to care.
- Stay current on best practices and advancements in the field of case management and rehabilitation services.

- Advocate for the needs and rights of individuals with disabilities.
- Create a supportive and empowering environment for clients to facilitate their growth and development.
- Participate in team meetings and case conferences to discuss client progress and address any challenges.
- Provide support and guidance to clients as they navigate the rehabilitation process.
- Maintain confidentiality and respect the privacy of clients.
- Collaborate with community organizations and agencies to expand resources and services available to clients.
- Continuously evaluate and adjust intervention strategies to meet the changing needs and goals of clients.
- Serve as a positive role model and mentor for clients.
- Adhere to all agency policies and procedures.
- Complete all required training and professional development activities.
- Attend and participate in relevant conferences and workshops.
- Uphold the agency's mission and values in all interactions and activities.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
At Least 1 Year Of Experience In Case Management Or Social Services.
Knowledge Of Laws, Regulations, And Resources Related To Rehabilitation Services.
Ability To Develop And Implement Individualized Treatment Plans For Clients.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Clients, Families, And Service Providers.
