- Provide comprehensive social work services to veterans and their families at the Illinois Veterans Home at LaSalle (IVH-L).
- Develop and implement individualized treatment plans for veterans based on their specific needs and goals.
- Conduct assessments and evaluations to determine the mental health needs of veterans.
- Collaborate with interdisciplinary teams and external agencies to coordinate services and resources for veterans.
- Advocate for veterans' rights and ensure they have access to necessary services and benefits.
- Provide crisis intervention and support to veterans in need.
- Maintain accurate and up-to-date documentation of all interactions and services provided to veterans.
- Stay informed and knowledgeable about the Illinois Department of Human Services' Division of Mental Health policies and procedures.
- Monitor and evaluate the effectiveness of treatment plans and make necessary adjustments.
- Participate in continuing education and training opportunities to enhance knowledge and skills.
- Communicate effectively and professionally with veterans, families, colleagues, and other stakeholders.
- Maintain confidentiality and adhere to ethical standards in all interactions and decisions.
- Attend meetings and conferences to represent IVH-L and stay updated on best practices in the field of social work.
- Serve as a positive role model and promote a supportive and inclusive environment for veterans.
- Collaborate with community organizations and resources to provide additional support and services for veterans.
- Complete all required reports and documentation accurately and in a timely manner.
- Stay current on relevant laws and regulations related to social work and mental health services.
- Provide guidance and support to other staff members and interns as needed.
- Participate in quality improvement initiatives and contribute to the overall mission and goals of IVH-L.
- Serve as an advocate for the importance of mental health and social work services for veterans.
Bachelor's Or Master's Degree In Social Work From An Accredited University.
Minimum Of 2 Years Of Experience In Social Work, Preferably In A Healthcare Or Mental Health Setting.
Valid License As A Licensed Social Worker (Lsw) Or Licensed Clinical Social Worker (Lcsw) In The State Of Illinois.
Knowledge And Understanding Of The Illinois Mental Health And Developmental Disabilities Code And Other Relevant Laws And Regulations.
Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With Diverse Populations And Collaborate With Interdisciplinary Teams.
