In this role, you will play a critical role in coordinating and overseeing the delivery of habilitation services to individuals with developmental disabilities.   1. Coordinate and oversee the delivery of habilitation services to individuals with developmental disabilities.
  2. Develop and maintain effective relationships with community partners and service providers to ensure the highest quality of care for program participants.
  3. Monitor and evaluate the effectiveness of habilitation services and make recommendations for improvement.
  4. Collaborate with interdisciplinary teams to develop individualized habilitation plans for program participants.
  5. Ensure compliance with all state and federal regulations and standards for habilitation services.
  6. Train and supervise program staff to ensure the delivery of high-quality, person-centered care.
  7. Manage program budget and resources effectively to maximize impact and efficiency.
  8. Advocate for the rights and needs of individuals with disabilities and promote inclusion and independence.
  9. Stay updated on best practices and research in the field of habilitation services and incorporate these into program development.
  10. Maintain accurate and complete records and reports related to program services, participants, and outcomes.
  11. Participate in meetings, conferences, and trainings related to program coordination and disability services.
  12. Represent the program and agency in a positive and professional manner to external stakeholders.
  13. Continuously assess and address the changing needs and challenges of individuals with disabilities and the habilitation services system.
  14. Collaborate with other agencies and departments to coordinate services and resources for program participants.
  15. Ensure the safety and well-being of program participants at all times.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Minimum Of 3 Years Of Experience In Program Coordination Or Management Within A Human Services Setting.

  • Knowledge Of State And Federal Regulations Related To Habilitation Services For Individuals With Disabilities.

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Multiple Stakeholders And Facilitate Group Discussions.

  • Experience With Budget Management, Grant Writing, And Data Analysis.
