Care Coordinator - Children's Behavioral Health Home

As a Care Coordinator, you will work with children and their families to address behavioral health needs. You will provide intake and complete assessment process when clients are referred and develop a plan of care for the child with input from the team that integrates behavioral and physical health needs and goals. You will oversee implementation of the plan and monitor progress. You will connect families with needed resources, address barriers, and make referrals. You will work closely with our school partners, and potentially co-locate within a school to enhance collaboration and teamwork.
