
Risk Adjustment Program Lead - Population Health
Saint Francis Health System is looking for a highly organized and experienced individual to take on the role of Risk Adjustment Program Lead - Population Health. The successful candidate will be an exceptional leader with a passion for developing and delivering high-quality population health programs that improve patient outcomes.The Risk Adjustment Program Lead - Population Health will be responsible for oversight and coordination of all activities related to population health, including risk adjustment and quality improvement initiatives. The ideal candidate will have a deep understanding of healthcare operations, population health, data analysis and reporting, and risk adjustment principles and practices.The ideal candidate will possess: -A Bachelor’s Degree in healthcare administration, public health, or a related field -At least 5 years of experience in healthcare operations, population health, and risk adjustment -Project management experience and the ability to manage multiple initiatives -Excellent communication and interpersonal skills -Strong organizational skills and the ability to prioritize tasks -An analytical mindset and the ability to interpret and utilize data -Knowledge of healthcare regulations and compliance standards If you have a passion for population health and a commitment to improving patient outcomes, we would love to hear from you.
Responsibilities:
- Develop and deliver high-quality population health programs to improve patient outcomes
- Oversee and coordinate all activities related to population health, including risk adjustment and quality improvement initiatives
- Utilize healthcare operations, population health, data analysis and reporting, and risk adjustment principles and practices
- Manage multiple initiatives and prioritize tasks accordingly
- Interpret and utilize data to assess population health initiatives
- Ensure healthcare regulations and compliance standards are met
- Provide excellent communication and interpersonal skills to collaboratively work with stakeholders and other team members.
