- Develop and implement care plans for patients and their families to address their social, emotional, and practical needs.
- Conduct comprehensive psychosocial assessments to identify patients' and families' needs and concerns.
- Provide counseling and support to patients and families during difficult times, including illness, death, and end-of-life care.
- Collaborate with healthcare team members to ensure coordinated and holistic care for patients.
- Advocate for patients' rights and access to resources and services.
- Connect patients and families with community resources, such as financial assistance, housing, and support groups.
- Maintain accurate and up-to-date documentation in patient records.
- Provide education and support to patients and families regarding healthcare systems and options.
- Participate in interdisciplinary meetings and case conferences to discuss patient care and treatment plans.
- Stay informed about current social work practices and policies, and incorporate them into daily practice.
- Develop and maintain relationships with community organizations and agencies to provide additional support and resources to patients and families.
- Collaborate with discharge planners and insurance companies to ensure appropriate transitions of care for patients.
- Provide crisis intervention and support to patients and families in emergency situations.
- Adhere to ethical and professional standards of social work practice.
- Continuously evaluate the effectiveness of interventions and make necessary adjustments to improve patient care.
Master's Degree In Social Work (Msw) From An Accredited Program.
Current State-Issued Social Work License.
Minimum Of 2 Years Of Clinical Social Work Experience In A Healthcare Setting, Preferably In A Hospital Or Medical Facility.
Knowledge And Experience In A Variety Of Social Work Interventions And Techniques, Including Crisis Intervention, Case Management, And Advocacy.
Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Collaborate With Diverse Populations, Including Patients, Families, And Healthcare Professionals.
