Job Description

We are looking for an experienced and compassionate Senior Care Management Coordinator to join the team at Providence St. Patrick Hospital. In this role, you will be responsible for providing support and coordination for our Senior Care Services department. You will serve as the primary contact for patient and family members, coordinate care plans, and provide resources and guidance for clinical staff.To be successful in this position, you should have a thorough knowledge of senior care services, an understanding of community resources, and excellent communication and interpersonal skills. The ideal candidate will also be highly organized, detail-oriented, and able to work well independently. Responsibilities:

  1. Serve as the primary contact for patient and family members, providing support and responding to inquiries in a timely and professional manner.
  2. Coordinate care plans for seniors, ensuring that the plans are comprehensive and tailored to individual needs.
  3. Monitor patient progress and provide resources and guidance for clinical staff.
  4. Analyze data to identify trends and areas of improvement in senior care services.
  5. Develop and implement strategies to optimize senior care services.
  6. Maintain accurate records of patient care, including medical history, treatments, medications, and progress.
  7. Research and identify community resources available to senior patients and their families.
  8. Collaborate with other department personnel to ensure effective communication and coordination of care.
  9. Provide education and training to healthcare professionals, caregivers, and family members on best practices in senior care services.
  10. Monitor compliance with relevant regulations and standards.

Job Qualifications
