RN - Care Manager

Responsibilities:

1. Manage and coordinate patient care services and activities to ensure best outcomes for patients.
2. Maintain current RN license and stay up to date on latest trends in healthcare.
3. Provide quality healthcare and customer service to all patients and their families.
4. Develop and implement patient care plans.
5. Monitor treatment progress and evaluate patient outcomes.
6. Liaise with physicians, nurses, and other healthcare professionals to coordinate efficient and effective care.
7. Document patient care services and activities in patient medical records.
8. Educate patients and families on disease prevention and treatment.
9. Act as a patient advocate and ensure patient rights are respected.
10. Ensure compliance with all applicable laws and regulations.
