
Registered Nurse Heart Transplant Coordinator

- Coordinate and manage all aspects of heart transplant patients' care, including pre-transplant evaluation and post-transplant follow-up.
- Collaborate with physicians, surgeons, and other healthcare team members to ensure seamless and high-quality care for patients.
- Develop and implement care plans specific to each heart transplant patient, taking into consideration their unique needs and medical history.
- Monitor patients' progress and provide ongoing education and support to patients and their families throughout the transplant process.
- Collaborate with insurance companies and other healthcare facilities to ensure appropriate coverage and coordination of care.
- Maintain accurate and thorough documentation of all patient interactions and interventions.
- Adhere to all hospital policies, procedures, and protocols related to heart transplant care.
- Stay up-to-date on the latest advancements and best practices in heart transplant care, and apply this knowledge to improve patient outcomes.
- Participate in interdisciplinary team meetings and contribute to the development of new protocols and processes to improve the heart transplant program.
- Serve as a resource and mentor for other nursing staff, providing guidance and support in the care of heart transplant patients.
- Collaborate with the transplant team to ensure timely and appropriate donor-recipient matching.
- Maintain a safe and clean work environment, following infection control and safety protocols at all times.
- Foster a compassionate and supportive environment for patients and their families, providing emotional support and guidance as needed.
- Adhere to all legal and ethical standards in the care of heart transplant patients.
- Continuously assess and evaluate the effectiveness of care plans and make necessary adjustments to ensure optimal patient outcomes.
- Exhibit strong critical thinking skills and the ability to make quick and accurate decisions in emergency situations.
- Communicate effectively with patients, families, and healthcare team members, providing thorough and timely updates on patients' conditions.

Bachelor's Degree In Nursing From An Accredited Institution.
Current, Unrestricted Registered Nurse (Rn) License In The State Of Montana.
Minimum Of 2 Years Of Experience In Critical Care Or Cardiac Nursing.
Certification From The American Association Of Heart Failure Nurses (Aahfn) Or The American Association Of Critical-Care Nurses (Aacn).
