  • Conduct thorough assessments of patients' health and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patients' health and provide necessary interventions and treatments
  • Educate patients and their families on proper healthcare practices and disease management
  • Document and maintain accurate and up-to-date patient records
  • Collaborate with interdisciplinary team members to provide comprehensive care
  • Communicate effectively with patients, families, and healthcare professionals

Job Qualifications
  • Active Lpn License In The State Of Montana.

  • Minimum Of 1 Year Experience In Home Health Or Related Field.
