- Conduct thorough assessments of patients' health and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' health and provide necessary interventions and treatments
- Educate patients and their families on proper healthcare practices and disease management
- Document and maintain accurate and up-to-date patient records
- Collaborate with interdisciplinary team members to provide comprehensive care
- Communicate effectively with patients, families, and healthcare professionals
- Maintain a safe and comfortable environment for patients
- Ensure compliance with all regulatory and safety standards
- Participate in ongoing education and training to enhance skills and knowledge

Active Lpn License In The State Of Montana.
Minimum Of 1 Year Experience In Home Health Or Related Field.
Knowledge Of Medicare And Medicaid Regulations And Guidelines.
Strong Communication And Documentation Skills.
Ability To Work Independently And In A Team Environment.
