
LPN - Home Health

We are currently seeking a compassionate and skilled LPN to join our Home Health team. - Provide exceptional care to patients in their homes, following all established protocols and procedures.
- Utilize nursing skills and expertise to assess, plan, implement, and evaluate patient care.
- Monitor patients' physical and emotional well-being, and communicate any changes or concerns to the appropriate healthcare providers.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients.
- Educate patients and their families on their health conditions, treatments, and self-care techniques.
- Maintain accurate and up-to-date patient records and documentation.
- Follow all safety and infection control protocols to ensure the well-being of patients.
- Provide emotional support and comfort to patients and their families during difficult times.
- Participate in ongoing training and professional development opportunities to improve skills and knowledge.
- Adhere to all legal and ethical standards of nursing practice.
- Serve as a resource and mentor to other members of the healthcare team.
- Represent the hospital and home health department in a professional and compassionate manner.
- Continuously strive to provide the highest quality of care and customer service to patients and their families.
- Act as an advocate for patients to ensure their needs and preferences are being met.
Valid Lpn License: Candidates Must Possess A Valid And Current Lpn License In The State Of Montana To Be Considered For The Position.
Minimum Of 1 Year Experience In Home Health: The Ideal Candidate Should Have At Least 1 Year Of Experience Working As A Lpn In A Home Health Setting.
Proficiency In Patient Care: Candidates Must Have A Strong Understanding Of Patient Care And Be Able To Provide Quality Care To Patients In Their Homes.
Excellent Communication Skills: Strong Communication Skills Are Essential For This Role, As Lpns Will Be Required To Communicate With Patients, Their Families, And Other Healthcare Professionals.
Ability To Work Independently: Home Health Lpns Must Be Self-Motivated And Able To Work Independently, As They Will Often Be The Primary Healthcare Provider For Their Patients.
