LPN - Home Health

1. Provide exceptional care to patients in their homes, following all established protocols and procedures.
2. Utilize nursing skills and expertise to assess, plan, implement, and evaluate patient care.
3. Monitor patients' physical and emotional well-being, and communicate any changes or concerns to the appropriate healthcare providers.
4. Administer medications and treatments as prescribed by physicians.
5. Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients.
6. Educate patients and their families on their health conditions, treatments, and self-care techniques.
7. Maintain accurate and up-to-date patient records and documentation.
8. Follow all safety and infection control protocols to ensure the well-being of patients.
9. Provide emotional support and comfort to patients and their families during difficult times.
10. Participate in ongoing training and professional development opportunities to improve skills and knowledge.
11. Adhere to all legal and ethical standards of nursing practice.
12. Serve as a resource and mentor to other members of the healthcare team.
13. Represent the hospital and home health department in a professional and compassionate manner.
14. Continuously strive to provide the highest quality of care and customer service to patients and their families.
15. Act as an advocate for patients to ensure their needs and preferences are being met.
