- Provide high-quality care to patients in their homes in accordance with established protocols and standards of practice.
- Administer medications and treatments as prescribed by physicians.
- Conduct thorough patient assessments and develop individualized care plans.
- Monitor and report changes in patients' conditions to the appropriate healthcare providers.
- Educate patients and their families on health management and disease prevention.
- Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care.
- Maintain accurate and up-to-date patient records and documentation.
- Adhere to all regulatory and legal requirements, as well as organizational policies and procedures.
- Communicate effectively with patients, families, and team members to ensure a positive and supportive healthcare experience.
- Participate in ongoing training and development opportunities to enhance clinical skills and knowledge.
- Adhere to ethical and professional standards of conduct at all times.

Excellent Communication And Interpersonal Skills.
Current Lpn Licensure In The State Of Montana.
Minimum Of 1 Year Experience In Home Health Or A Related Field.
Knowledge Of Medicare And Medicaid Regulations.
Ability To Work Independently And Prioritize Tasks Effectively.
