Job Description

We are seeking an experienced and compassionate Licensed Practical Nurse to join our Hospice team at Providence St. Patrick Hospital. The successful candidate will provide holistic, family-centered care to patients and their families who are facing terminal illness. This individual will demonstrate a commitment to the delivery of quality care and must possess a strong sense of empathy and understanding.The ideal candidate will have a current LPN license in Montana and a minimum of one year of experience in hospice, home health, or long-term care. The preferred candidate will also have a minimum of two years of experience in a health care setting and a strong knowledge of end-of-life care. BLS certification is required.

Responsibilities:

  1. Provide holistic, patient-centered care to patients and their families facing terminal illness.
  2. Utilize a strong sense of empathy and understanding when interacting with patients and their families.
  3. Administer medications, treatments, and other nursing care as prescribed by the physician.
  4. Monitor patient condition, document changes, and report any concerns or changes to the physician.
  5. Keep accurate records of patient care, including vital signs, medications, treatments, and progress notes.
  6. Maintain complete patient confidentiality in accordance with HIPAA regulations.
  7. Educate patients and families on end-of-life care options.
  8. Attend regular meetings with the hospice team to review patient status and plan for care.
  9. Participate in ongoing professional development activities to stay current on best practices in hospice care.
  10. Maintain a current LPN license in Montana.
