
Licensed Practical Nurse - Hospice
We are seeking an experienced and compassionate Licensed Practical Nurse to join our Hospice team at Providence St. Patrick Hospital. The successful candidate will provide holistic, family-centered care to patients and their families who are facing terminal illness. This individual will demonstrate a commitment to the delivery of quality care and must possess a strong sense of empathy and understanding.The ideal candidate will have a current LPN license in Montana and a minimum of one year of experience in hospice, home health, or long-term care. The preferred candidate will also have a minimum of two years of experience in a health care setting and a strong knowledge of end-of-life care. BLS certification is required.
Responsibilities:
- Provide holistic, patient-centered care to patients and their families facing terminal illness.
- Utilize a strong sense of empathy and understanding when interacting with patients and their families.
- Administer medications, treatments, and other nursing care as prescribed by the physician.
- Monitor patient condition, document changes, and report any concerns or changes to the physician.
- Keep accurate records of patient care, including vital signs, medications, treatments, and progress notes.
- Maintain complete patient confidentiality in accordance with HIPAA regulations.
- Educate patients and families on end-of-life care options.
- Attend regular meetings with the hospice team to review patient status and plan for care.
- Participate in ongoing professional development activities to stay current on best practices in hospice care.
- Maintain a current LPN license in Montana.
