Home Health Aide

We are looking for a compassionate and dedicated Home Health Aide to join our team at Providence St. Patrick Hospital. Our Home Health Aide will provide assistance to patients who are residing in their own homes, nursing homes, or assisted living facilities. You will be an important part of our team, responsible for ensuring that our patients receive the best care possible. This is an excellent opportunity for someone with a strong commitment to caring for others and the ability to make a positive difference in the lives of our patients. To be successful in this role, you will need to be reliable, organized, and possess strong communication skills.The ideal candidate should have at least one year of experience as a Home Health Aide, a current certification as a Home Health Aide, and knowledge of home health care standards and regulations. In addition, you must have a valid driver’s license and access to a reliable vehicle.

Responsibilities:

1. Provide assistance to patients in their own homes, nursing homes, or assisted living facilities.
2. Ensure that patients receive the best care possible.
3. Maintain a reliable and organized work schedule.
4. Communicate effectively with patients and their families.
5. Administer medications and other treatments as prescribed by a physician.
6. Provide emotional and physical support to patients and their families.
7. Demonstrate knowledge of home health care standards and regulations.
8. Maintain a valid driver’s license and access to a reliable vehicle.
9. Document patient care activities.
10. Monitor patient’s condition and report changes to the supervising nurse.
