Job Description

Providence St. Patrick Hospital is seeking an experienced Certified Nursing Assistant (CNA) to join our Transitional Care team. The ideal candidate will have a strong commitment to providing exemplary patient care and the ability to work in a fast-paced environment. The CNA will be responsible for providing compassionate care to patients who are transitioning from hospital care to long-term care or home. This role requires exceptional communication and interpersonal skills, as well as a thorough understanding of the nursing process. The successful candidate will possess the following qualifications:• Current Certified Nursing Assistant Certification• Strong patient care experience in an acute care setting• Excellent communication and organizational skills• Ability to work collaboratively with all members of the healthcare team• Knowledge of medical terminology• Compassionate and patient-centric approach to careIf you possess the qualifications and have the desire to provide the highest quality of care to our patients, we invite you to apply today.

Responsibilities:

  1. Provide direct patient care and support services to patients who are in transition from hospital care to long-term care or home.
  2. Follow all established protocols to ensure patient safety and comfort.
  3. Assist with patient transfers, bathing, grooming, and other activities of daily living.
  4. Administer medications and treatments as ordered by the physician.
  5. Document patient care in the electronic health records system.
  6. Monitor vital signs and report any changes to the RN or physician.
  7. Provide patient education and support to families and caregivers.
  8. Assist with admission and discharge processes.
  9. Participate in interdisciplinary team meetings to ensure continuity of care.
  10. Maintain a clean and safe environment for patients, visitors and staff.
