
CNA - Transitional Care
Providence St. Patrick Hospital is seeking an experienced Certified Nursing Assistant (CNA) to join our Transitional Care team. The ideal candidate will have a strong commitment to providing exemplary patient care and the ability to work in a fast-paced environment. The CNA will be responsible for providing compassionate care to patients who are transitioning from hospital care to long-term care or home. This role requires exceptional communication and interpersonal skills, as well as a thorough understanding of the nursing process. Responsibilities:
- Provide direct patient care and support services to patients who are in transition from hospital care to long-term care or home.
- Follow all established protocols to ensure patient safety and comfort.
- Assist with patient transfers, bathing, grooming, and other activities of daily living.
- Administer medications and treatments as ordered by the physician.
- Document patient care in the electronic health records system.
- Monitor vital signs and report any changes to the RN or physician.
- Provide patient education and support to families and caregivers.
- Assist with admission and discharge processes.
- Participate in interdisciplinary team meetings to ensure continuity of care.
- Maintain a clean and safe environment for patients, visitors and staff.
Certified Nursing Assistant (Cna) Certification
Experience In Providing Transitional Care To Post-Acute Patients
