- Dispensing and reviewing prescribed medications to ensure accuracy and patient safety
- Collaborating with physicians and other healthcare professionals to provide individualized treatment plans for patients
- Conducting medication therapy management and monitoring for patients with chronic conditions
- Participating in interdisciplinary rounds and providing valuable input on medication-related matters
- Keeping up-to-date with the latest pharmaceutical advancements and incorporating them into patient care
- Ensuring compliance with all state and federal regulations and maintaining accurate records
- Providing patient education on medication usage, side effects, and potential interactions
- Identifying and addressing medication errors and adverse drug reactions
- Assisting with medication reconciliation during transitions of care
- Contributing to quality improvement initiatives and participating in pharmacy-related committees
- Mentoring and training pharmacy students and residents
- Maintaining a high level of professionalism and ethical standards in all interactions with patients and colleagues.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Active And Unrestricted Pharmacist License In The State Of Montana.
Completion Of A Postgraduate Residency Program In Clinical Pharmacy Or Equivalent Experience.
Strong Clinical Knowledge And Experience In A Hospital Or Acute Care Setting.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Healthcare Teams.
