• Dispensing and reviewing prescribed medications to ensure accuracy and patient safety
• Collaborating with physicians and other healthcare professionals to provide individualized treatment plans for patients
• Conducting medication therapy management and monitoring for patients with chronic conditions
• Participating in interdisciplinary rounds and providing valuable input on medication-related matters
• Keeping up-to-date with the latest pharmaceutical advancements and incorporating them into patient care
• Ensuring compliance with all state and federal regulations and maintaining accurate records
• Providing patient education on medication usage, side effects, and potential interactions
• Identifying and addressing medication errors and adverse drug reactions
