
Clinical Pharmacist

- Collaborate with physicians and other healthcare professionals to review and optimize patients' medication therapy plans.
- Conduct thorough medication reviews and make recommendations for adjustments as needed.
- Ensure accurate and timely dispensing of medications to patients.
- Maintain knowledge of current pharmaceutical developments and trends to provide evidence-based care.
- Participate in patient care rounds and provide input on medication-related issues.
- Educate patients and caregivers on proper medication administration, potential side effects, and drug interactions.
- Oversee the management and monitoring of medication inventory and ensure compliance with regulatory standards.
- Communicate effectively with patients, healthcare team members, and external partners.
- Stay up-to-date on insurance coverage and reimbursement policies to assist patients in accessing necessary medications.
- Collaborate with the pharmacy team to ensure accurate and timely billing of medications.
- Develop and implement protocols and procedures to improve medication safety and patient outcomes.
- Participate in quality improvement initiatives and assist with data collection and analysis.
- Maintain accurate and complete patient records.
- Adhere to all hospital policies, procedures, and regulatory requirements.
- Act as a resource for other healthcare professionals and provide guidance on medication-related issues.
- Continuously evaluate and improve pharmaceutical care processes.
- Stay informed of changes in the healthcare landscape and adapt practice accordingly.
- Maintain confidentiality and adhere to ethical standards in all aspects of practice.
- Attend department meetings and participate in continuing education opportunities to enhance professional development.
- Maintain a positive and professional attitude and uphold the mission and values of the hospital.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Minimum Of 2 Years Of Experience In A Clinical Pharmacy Setting.
Current State Licensure As A Pharmacist In Montana.
Board Certification In A Relevant Specialty (E.g. Pharmacotherapy, Ambulatory Care).
Strong Clinical Knowledge And Ability To Provide Evidence-Based Pharmaceutical Care To Patients.
