Job Description

We are seeking a dedicated and compassionate Clinical Pharmacist to join our team at Providence St. Patrick Hospital. As a Clinical Pharmacist, you will play a vital role in providing exceptional patient care by collaborating with physicians and other healthcare professionals to ensure appropriate medication therapy.   1. Collaborate with physicians and other healthcare professionals to review and optimize patients' medication therapy plans.
  2. Conduct thorough medication reviews and make recommendations for adjustments as needed.
  3. Ensure accurate and timely dispensing of medications to patients.
  4. Maintain knowledge of current pharmaceutical developments and trends to provide evidence-based care.
  5. Participate in patient care rounds and provide input on medication-related issues.
  6. Educate patients and caregivers on proper medication administration, potential side effects, and drug interactions.
  7. Oversee the management and monitoring of medication inventory and ensure compliance with regulatory standards.
  8. Communicate effectively with patients, healthcare team members, and external partners.
  9. Stay up-to-date on insurance coverage and reimbursement policies to assist patients in accessing necessary medications.
  10. Collaborate with the pharmacy team to ensure accurate and timely billing of medications.
  11. Develop and implement protocols and procedures to improve medication safety and patient outcomes.
  12. Participate in quality improvement initiatives and assist with data collection and analysis.
  13. Maintain accurate and complete patient records.
  14. Adhere to all hospital policies, procedures, and regulatory requirements.
  15. Act as a resource for other healthcare professionals and provide guidance on medication-related issues.
  16. Continuously evaluate and improve pharmaceutical care processes.
  17. Stay informed of changes in the healthcare landscape and adapt practice accordingly.
  18. Maintain confidentiality and adhere to ethical standards in all aspects of practice.
  19. Attend department meetings and participate in continuing education opportunities to enhance professional development.
  20. Maintain a positive and professional attitude and uphold the mission and values of the hospital.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.

  • Minimum Of 2 Years Of Experience In A Clinical Pharmacy Setting.

  • Current State Licensure As A Pharmacist In Montana.

  • Board Certification In A Relevant Specialty (E.g. Pharmacotherapy, Ambulatory Care).

  • Strong Clinical Knowledge And Ability To Provide Evidence-Based Pharmaceutical Care To Patients.
