
Clinical Liaison RN
We are looking for a compassionate and experienced Clinical Liaison RN to join our team at Providence St. Patrick Hospital! Our ideal candidate is an excellent communicator, highly organized, and driven to provide exceptional care to our patients and their families. The Clinical Liaison RN will be responsible for building relationships with our referral sources, managing the referral process, and ensuring a smooth transition of care from referral source to hospital.The successful candidate will have a valid RN license, an understanding of clinical operations, and a minimum of three years of clinical experience in an acute care setting. Additionally, the Clinical Liaison RN should be knowledgeable about current healthcare trends and regulations, have strong problem-solving skills, and demonstrate exceptional customer service and interpersonal skills. Responsibilities of a Clinical Liaison RN:
- Build relationships with referral sources to facilitate smooth transitions of care.
- Manage the referral process for patients and their families.
- Maintain valid RN license and remain updated on healthcare trends and regulations.
- Provide exceptional customer service and interpersonal skills.
- Utilize problem-solving skills to address patient issues.
- Ensure quality care to patients and their families.
- Educate referral sources on the services provided by Providence St. Patrick Hospital.
- Track and manage referrals to ensure timely communication and follow-up.
- Assist in resolving any issues related to the referral process.
- Participate in hospital meetings and other clinical activities as needed.
