Job Description

We are seeking an experienced Case Management Associate to join our team at Providence St. Patrick Hospital. The ideal candidate for this position will have a passion for delivering excellent patient care and a commitment to providing top-notch customer service. As a part of the hospital's team, the Case Management Associate will play a key role in ensuring the highest quality of care for our patients.The successful candidate must have a Bachelor's degree in healthcare or related field, at least 3 years of experience in a healthcare setting, and a strong knowledge of medical terminology, insurance policies, and case management processes. The ability to work independently and in teams, excellent organizational skills, and a strong attention to detail are essential. The successful candidate must also have excellent interpersonal and customer service skills and the ability to interact with patients, families, and staff in a professional and courteous manner.

Responsibilities:

  1. Ensure that the highest quality of care is provided to patients by implementing best practices in case management processes.
  2. Provide excellent customer service to patients and families.
  3. Maintain detailed records of patient care plans and outcomes.
  4. Monitor and document patient progress.
  5. Assist in establishing individualized treatment plans.
  6. Collaborate with other healthcare professionals to coordinate patient care.
  7. Attend case management meetings and team meetings.
  8. Ensure that insurance policies are adhered to.
  9. Provide patient education and resources.
  10. Ensure compliance with all applicable laws and regulations.
