  1. Provide emotional support and counseling to patients and their families who are affected by infectious diseases.
  2. Collaborate with interdisciplinary teams to develop individualized care plans for patients.
  3. Conduct assessments to identify patients' social, emotional, and financial needs.
  4. Assist patients in accessing resources and services, such as financial assistance, housing, and transportation.
  5. Educate patients and their families on managing their condition and navigating healthcare systems.
  6. Advocate for patients' rights and needs within the healthcare system.
  7. Develop and maintain relationships with community organizations and resources to provide additional support for patients.
  8. Document patient interactions and progress in electronic medical records.
  9. Stay up-to-date on current trends and best practices in the field of social work and infectious diseases.
  10. Participate in team meetings and contribute to the development of policies and procedures related to patient care.
  11. Maintain confidentiality and adhere to professional ethical standards.
  12. Communicate effectively with patients, families, and healthcare team members.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited University.

  • Minimum Of 2 Years Of Experience Working In The Field Of Infectious Diseases Or Public Health.

  • Knowledge Of Local, State, And Federal Resources And Programs Related To Infectious Diseases.

  • Ability To Provide Culturally Competent Care And Support To Diverse Populations Affected By Infectious Diseases.

  • Strong Communication, Organizational, And Problem-Solving Skills, As Well As The Ability To Work Effectively In A Multidisciplinary Team.
