
Registered Nurse (RN) - Transplant Coordinator

- Provide exceptional care and support to patients undergoing the transplant process.
- Serve as the primary point of contact for patients, families, and healthcare providers during the transplant journey.
- Educate patients and their families on the transplant process, including pre- and post-operative care.
- Collaborate with a multidisciplinary team to develop individualized care plans for patients.
- Monitor and assess patients' physical and emotional well-being before, during, and after the transplant procedure.
- Administer medications and treatments as prescribed by physicians.
- Coordinate and schedule appointments, tests, and procedures for patients.
- Keep accurate and up-to-date records of patients' progress and procedures.
- Communicate effectively with patients, families, and healthcare providers to ensure a smooth and efficient transplant process.
- Stay current with advancements and best practices in transplant care and participate in continuing education opportunities.
- Adhere to all organizational policies, procedures, and protocols.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Act as an advocate for patients and their families, ensuring their needs and concerns are addressed.
- Assist in the development and implementation of quality improvement initiatives related to transplant care.
- Participate in on-call rotation and be available for emergency situations as needed.

Bachelor's Or Master's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing (Bsn) Is Required For A Registered Nurse (Rn) - Transplant Coordinator Position At Prisma Health. A Master's Degree In Nursing (Msn) May Be Preferred For More Advanced Roles.
Current Rn License: All Candidates Must Possess A Current And Active Registered Nurse (Rn) License In The State Of South Carolina. The License Must Be In Good Standing And Without Any Restrictions Or Disciplinary Actions.
Previous Transplant Experience: Candidates Should Have Previous Experience Working In A Transplant Setting, Ideally In A Coordinator Role. This May Include Experience In Organ Procurement, Transplant Surgery, Or Post-Transplant Care.
Strong Communication And Organizational Skills: As A Transplant Coordinator, Effective Communication And Organization Are Essential For Coordinating Complex Care Plans And Collaborating With Multiple Healthcare Providers. Candidates Should Possess Excellent Interpersonal Skills And The Ability To Manage Multiple Tasks Simultaneously.
