Welcome to Prisma Health! We are currently seeking a highly motivated and compassionate Registered Nurse (RN)-Intake Coordinator to join our SeniorCare PACE team. In this role, you will play a crucial role in coordinating and managing the intake process for our Program of All-Inclusive Care for the Elderly (PACE) participants. As a PACE Intake Coordinator, you will have the opportunity to make a positive impact on the lives of our senior community by providing them with comprehensive and coordinated care. We are looking for an individual who is passionate about working with the elderly population and has a strong clinical background.   1. Coordinate the intake process for all PACE participants, ensuring timely and efficient enrollment into the program.
  2. Communicate with potential participants and their families to provide information about the PACE program and answer any questions.
  3. Collaborate with the PACE interdisciplinary team to evaluate potential participants and determine eligibility for the program.
  4. Conduct initial assessments and gather necessary information to develop individualized care plans for each participant.
  5. Maintain accurate and up-to-date records of all PACE enrollment activities.
  6. Serve as a liaison between PACE participants, their families, and the PACE team to ensure seamless and coordinated care.
  7. Educate participants and their families on the services and benefits provided by the PACE program.
  8. Monitor and track participant enrollment and ensure all required documentation is completed and submitted in a timely manner.
  9. Participate in team meetings and provide input on the development and implementation of care plans.
  10. Maintain confidentiality of all participant information in accordance with HIPAA regulations.
  11. Collaborate with community resources to ensure participants have access to necessary services and support.
  12. Continuously monitor and evaluate the effectiveness of the intake process and make recommendations for improvement.
  13. Maintain a positive and professional attitude and represent Prisma Health and the PACE program in a positive manner.
  14. Stay updated on industry trends and best practices related to the care of the elderly population.
  15. Provide exceptional customer service to all PACE participants and their families.

Job Qualifications
  • Current And Active Registered Nurse (Rn) License In The State Of South Carolina.

  • Bachelor's Degree In Nursing (Bsn) Or Equivalent From An Accredited Institution.

  • Minimum Of 2 Years Of Experience As A Registered Nurse In A Clinical Setting, Preferably In Geriatric Care.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With A Multidisciplinary Team.

  • Knowledge Of Medicare And Medicaid Regulations, As Well As Experience Working With Pace (Programs Of All-Inclusive Care For The Elderly) Participants.
