  1. Develop and maintain a comprehensive understanding of HIV and infectious diseases, including current treatment methods and resources.
  2. Provide case management services to patients with HIV and infectious diseases, including assessing their needs, developing care plans, and coordinating services.
  3. Collaborate with healthcare providers and community resources to ensure patients receive appropriate and timely care.
  4. Conduct regular assessments and evaluations to monitor patients' progress and adjust care plans as needed.
  5. Educate patients and their families about their conditions and treatment options.
  6. Maintain accurate and up-to-date documentation of patient interactions and care plans.
  7. Serve as an advocate for patients, ensuring they receive quality care and have access to necessary resources.
  8. Stay informed about changes and updates in the field of HIV and infectious diseases, and incorporate new information into practice.
  9. Collaborate with interdisciplinary team members to ensure continuity of care for patients.
  10. Participate in quality improvement initiatives to enhance the effectiveness of case management services.
  11. Maintain confidentiality and adhere to all HIPAA regulations.
  12. Communicate effectively with patients, colleagues, and other healthcare professionals.
  13. Attend trainings and conferences to enhance knowledge and skills related to HIV and infectious diseases.
  14. Adhere to all organizational policies and procedures.
  15. Demonstrate a commitment to providing exceptional healthcare services and promoting a positive work environment.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or A Related Field.

  • Minimum Of 3 Years Experience Working With Hiv And Infectious Disease Patients.

  • Strong Understanding Of Hiv Prevention, Treatment, And Care Protocols.

  • Knowledge Of Community Resources And Support Programs For Hiv Patients.

  • Excellent Communication And Interpersonal Skills To Collaborate With Multidisciplinary Teams And Advocate For Patients' Needs.
