1. Develop and implement health education programs and initiatives in collaboration with other team members.
2. Serve as a liaison with community organizations and stakeholders to promote health education and outreach efforts.
3. Conduct research and stay up-to-date on current health education trends and best practices.
4. Develop and maintain relationships with key community partners to facilitate effective program implementation.
5. Plan, coordinate, and deliver health education workshops, presentations, and events to community members.

7. Evaluate and analyze the effectiveness of health education programs and make recommendations for improvement.
8. Maintain accurate records and documentation of all program activities and outcomes.
9. Represent Prisma Health at community events and meetings to promote health education and outreach efforts.
10. Provide support and resources to individuals and families seeking information and resources related to health and wellness.
11. Communicate effectively with diverse populations and tailor health education materials and strategies to meet the needs of specific communities.
12. Participate in team meetings and contribute to the development of new strategies and initiatives to promote community health and well-being.
13. Collaborate with other health professionals, including nurses, physicians, and social workers, to provide comprehensive care to community members.
14. Adhere to all ethical and professional standards in the delivery of health education services.
