Job Description

  1. Develop and implement educational programs for patients with diabetes, including individualized care plans, group classes, and one-on-one counseling sessions.

  3. Educate patients on proper self-management techniques, including blood glucose monitoring, medication administration, and diet and exercise recommendations.
  4. Provide emotional support and encouragement to patients, helping them cope with the challenges of managing diabetes.
  5. Collaborate with other healthcare professionals to ensure coordinated and comprehensive care for patients with diabetes.
  6. Stay up-to-date on the latest research, trends, and treatments in diabetes care and incorporate this knowledge into patient education.
  7. Monitor and track patient progress and adjust care plans as needed to achieve optimal health outcomes.
  8. Serve as a resource for patients and their families, answering questions and providing guidance and support.
  9. Document all interactions and interventions accurately and in a timely manner.
  10. Participate in community outreach and education initiatives to raise awareness about diabetes prevention and management.
  11. Maintain confidentiality and adhere to all HIPAA regulations.
  12. Participate in ongoing training and professional development to expand knowledge and skills in diabetes care.
  13. Act as an advocate for patients, promoting self-advocacy and empowering them to take an active role in their healthcare.
  14. Collaborate with insurance companies and other payers to ensure coverage and reimbursement for diabetes education services.
  15. Adhere to all organizational policies and procedures and maintain a safe and clean work environment.

Job Qualifications
  • A Bachelor's Degree In Nursing, Nutrition, Or A Related Field.

  • Certification As A Certified Diabetes Educator (Cde) Or Willingness To Obtain Certification Within 1 Year Of Employment.

  • Minimum Of 2 Years Of Experience Working With Patients With Diabetes In A Clinical Or Educational Setting.

  • Strong Knowledge Of Diabetes Management, Treatment Options, And The Ability To Develop Individualized Care Plans.

  • Excellent Communication Skills And The Ability To Educate And Motivate Patients To Make Lifestyle Changes To Manage Their Diabetes.
