Job Description

We are seeking a compassionate and knowledgeable Diabetes Care Educator to join our team at Prisma Health.   1. Develop and implement educational programs and resources for patients with diabetes, including individual and group sessions, to promote understanding and self-management of the disease.
  2. Collaborate with healthcare providers and interdisciplinary teams to develop individualized care plans for patients with diabetes.
  3. Stay up-to-date on current research, treatments, and best practices in diabetes management to provide patients with the most accurate and relevant information.
  4. Provide ongoing support and counseling to patients with diabetes, addressing emotional and psychological aspects of the disease.
  5. Assess patients' understanding and adherence to treatment plans and make necessary adjustments to ensure optimal management of diabetes.
  6. Utilize effective communication skills to educate patients and their families on the importance of self-care and self-management.
  7. Organize and facilitate support groups and community events to promote awareness and education about diabetes.
  8. Collaborate with community resources and organizations to provide additional support and resources for patients with diabetes.
  9. Maintain accurate and thorough documentation of patient interactions and progress.
  10. Serve as a resource for colleagues and healthcare professionals on diabetes management and education.
  11. Adhere to all regulatory and compliance guidelines related to diabetes care and education.
  12. Continuously evaluate and improve educational programs and resources to ensure they meet the needs of patients and are aligned with industry standards.
  13. Participate in professional development opportunities to enhance knowledge and skills related to diabetes care and education.
  14. Demonstrate empathy, compassion, and sensitivity towards patients and their families.
  15. Adhere to the mission, vision, and values of Prisma Health in all interactions and responsibilities.

Job Qualifications
  • Bachelor's Degree In Nursing, Health Education, Or A Related Field.

  • Certified Diabetes Educator (Cde) Credential From The National Certification Board For Diabetes Educators (Ncbde).

  • Minimum Of 2-3 Years Of Experience In Diabetes Education, Preferably In A Healthcare Setting.

  • Proficient In Conducting Individual And Group Education Sessions, As Well As Developing And Implementing Diabetes Education Programs.

  • Strong Knowledge Of Diabetes Management, Including Medication, Nutrition, Exercise, And Self-Care Strategies.
