  1. Provide clinical expertise in medication therapy management for patients at Prisma Health.
  2. Collaborate with healthcare providers to develop and implement individualized medication plans for patients.
  3. Conduct medication reviews and make recommendations for adjustments or changes as necessary.
  4. Serve as a resource for the pharmacy team, providing guidance and support for complex medication-related issues.
  5. Stay current with industry developments and advancements in medication therapy to continually improve patient care.
  6. Participate in multidisciplinary rounds and meetings to discuss patient progress and medication management.
  7. Utilize evidence-based practices to promote safe and effective medication use.
  8. Monitor patient responses to medications and make appropriate interventions to optimize therapy.
  9. Educate patients and caregivers on medication use, side effects, and potential interactions.
  10. Maintain accurate and up-to-date patient records and documentation.
  11. Facilitate communication and collaboration between patients, healthcare providers, and other members of the healthcare team.
  12. Adhere to all state and federal regulations and guidelines for pharmacy practice.
  13. Participate in quality improvement initiatives to enhance patient outcomes.
  14. Serve as a preceptor for pharmacy students and residents.
  15. Attend continuing education programs and conferences to enhance professional development.
  16. Demonstrate a commitment to patient-centered care and exceptional customer service.
  17. Collaborate with the pharmacy team to ensure proper medication inventory and storage.
  18. Follow all safety protocols and procedures to maintain a safe working environment.
  19. Be an active and engaged member of the Prisma Health community, promoting the organization's mission and values.
  20. Perform other duties and responsibilities as assigned by the pharmacy manager or director.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.

  • Board Certification In A Specialized Area Of Pharmacy Practice, Such As Ambulatory Care, Critical Care, Or Oncology.

  • Minimum Of 3-5 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting.

  • Strong Clinical Knowledge And Expertise In Medication Therapy Management, Drug Interactions, And Patient Education.

  • Excellent Communication Skills And Ability To Collaborate With Interdisciplinary Teams To Improve Patient Outcomes.
