  1. Develop and maintain a comprehensive and evidence-based medication therapy plan for solid organ transplant patients.
  2. Collaborate with the transplant team to ensure seamless and coordinated care for patients.
  3. Provide accurate and timely medication education to patients and their families.
  4. Ensure compliance with all regulatory and accreditation standards related to medication management in transplant patients.
  5. Participate in rounds and multidisciplinary team meetings to provide pharmacotherapy recommendations and optimize patient outcomes.
  6. Conduct medication reconciliation and monitor for drug interactions, contraindications, and adverse effects.
  7. Serve as a clinical resource for other healthcare professionals on the transplant team.
  8. Stay updated on the latest advancements and best practices in transplant pharmacotherapy.
  9. Participate in quality improvement initiatives and research projects related to transplant pharmacotherapy.
  10. Act as a preceptor for pharmacy students and residents, providing guidance and mentorship.
  11. Maintain accurate and thorough documentation of patient care activities.
  12. Participate in the development and implementation of policies and protocols related to transplant pharmacotherapy.
  13. Serve as a liaison between the pharmacy department and other departments involved in transplant care.
  14. Demonstrate a commitment to patient safety and quality of care.
  15. Adhere to ethical and professional standards in all aspects of job performance.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School

  • Completion Of A Post-Graduate Residency Or Fellowship Program In Solid Organ Transplant Or A Related Specialty

  • Board Certification In Pharmacotherapy Or A Related Specialty (E.g. Bcps, Bcacp)

  • Minimum Of 2-3 Years Of Experience As A Clinical Pharmacist In A Solid Organ Transplant Setting

  • In-Depth Knowledge Of Immunosuppressive Medications And Their Interactions With Other Drugs Used In Transplant Patients
