  1. Provide specialized pharmaceutical care to solid organ transplant patients, including medication therapy management, drug interactions, and monitoring of drug levels.
  2. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans for transplant patients.
  3. Conduct medication reviews and make recommendations for adjustments to optimize patient outcomes.
  4. Keep up-to-date with the latest research and developments in solid organ transplant pharmacotherapy, and apply this knowledge to patient care.
  5. Maintain accurate and thorough patient records, including medication histories, drug therapy plans, and progress notes.
  6. Educate patients and their families on medication usage, side effects, and potential drug interactions.
  7. Participate in multi-disciplinary rounds and meetings to discuss patient care and treatment plans.
  8. Ensure compliance with all state and federal regulations related to pharmacy practice, including controlled substance laws.
  9. Provide guidance and support to other pharmacists and pharmacy technicians on solid organ transplant medication management.
  10. Collaborate with the pharmacy team to maintain accurate inventory levels and order medications as needed.
  11. Act as a resource for healthcare providers and staff on medication-related questions and concerns.
  12. Participate in quality improvement initiatives and contribute to the development of best practices for solid organ transplant care.
  13. Maintain professional competency through continuing education and training opportunities.
  14. Adhere to all hospital policies and procedures, including those related to patient safety and confidentiality.
