  1. Conduct comprehensive medication reviews to ensure appropriate prescribing, dosing, and administration of medications for patients.
  2. Collaborate with healthcare providers to develop individualized treatment plans for patients.
  3. Monitor patients for potential drug interactions, adverse reactions, and other medication-related issues.
  4. Evaluate and interpret laboratory results to make appropriate medication adjustments.
  5. Educate patients and caregivers on proper medication use, potential side effects, and the importance of adherence.
  6. Serve as a drug information resource for healthcare providers and patients.
  7. Participate in pharmacy and therapeutics committee meetings to review and make recommendations on formulary additions and revisions.
  8. Stay up-to-date on current pharmacy practices, guidelines, and regulations.
  9. Contribute to the development and implementation of medication-related policies and procedures.
  10. Participate in quality improvement initiatives to enhance patient safety and outcomes.
  11. Utilize technology and electronic health records to document patient care activities and communicate with healthcare team members.
  12. Provide drug therapy consultation for complex cases and serve as a resource for other pharmacists.
  13. Serve as a preceptor for pharmacy students and residents, providing education and training on clinical pharmacy practice.
  14. Maintain accurate and complete patient records and documentation.
  15. Adhere to all regulatory requirements and ethical standards in the delivery of pharmaceutical care.
  16. Participate in professional development activities and attend continuing education programs to maintain clinical knowledge and skills.
  17. Collaborate with other healthcare professionals to develop and implement interdisciplinary treatment plans.
  18. Act as a liaison between pharmacy, healthcare providers, and patients to ensure safe and effective medication use.
  19. Contribute to the development and implementation of medication-related protocols and guidelines.
  20. Assist with medication reconciliation and medication therapy management services as needed.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.

  • Board Certification In A Clinical Specialty Area, Such As Ambulatory Care Or Critical Care.

  • Current And Unrestricted Pharmacist License In The State Of South Carolina.

  • Minimum Of 2-3 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting.

  • Strong Knowledge And Understanding Of Evidence-Based Practice And Clinical Guidelines.
