  1. Provide exceptional pharmaceutical care to patients by ensuring accurate and safe medication delivery.
  2. Work closely with physicians and other healthcare professionals to develop and implement patient-specific treatment plans.
  3. Conduct thorough medication reviews and make recommendations for adjustments or changes as needed.
  4. Monitor patients for potential drug interactions, side effects, and efficacy of medications.
  5. Educate patients on proper use and potential side effects of prescribed medications.
  6. Collaborate with other healthcare team members to ensure comprehensive and coordinated patient care.
  7. Maintain accurate and up-to-date patient medication profiles.
  8. Stay current on new medications, treatment guidelines, and regulatory changes in the field of pharmacy.
  9. Participate in quality improvement initiatives to enhance patient outcomes and safety.
  10. Serve as a resource and mentor to other pharmacy staff, providing guidance and support as needed.
  11. Maintain strict adherence to all legal and ethical standards in pharmacy practice.
  12. Contribute to the development and implementation of departmental policies and procedures.
  13. Participate in training and development opportunities to continuously improve clinical skills and knowledge.
  14. Serve as a patient advocate by addressing any concerns or questions related to medication therapy.
  15. Foster a positive and collaborative work environment by promoting teamwork and effective communication.

Job Qualifications
  • Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited College Of Pharmacy.

  • Board Certification In A Specialized Area Of Pharmacy Practice, Such As Ambulatory Care Or Critical Care.

  • At Least 3-5 Years Of Experience Working As A Clinical Pharmacist In A Hospital Or Healthcare Setting.

  • Strong Knowledge Of Drug Therapy Guidelines, Medication Management Protocols, And Clinical Pharmacy Best Practices.

  • Excellent Communication Skills And Ability To Work Collaboratively With Interdisciplinary Healthcare Teams To Optimize Patient Outcomes.
