Bachelor's Degree In Social Work, Public Health, Or Related Field
Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting
Knowledge Of Infectious Diseases And Their Impact On Individuals And Communities
Familiarity With Local Housing Resources And Programs For Individuals With Infectious Diseases
Excellent Communication And Organizational Skills, With Ability To Work Collaboratively With Interdisciplinary Teams And Community Partners.
