  1. Provide case management services to individuals and families affected by infectious diseases and experiencing housing instability.
  2. Develop and maintain relationships with clients to assess their needs and create individualized plans for housing stability.
  3. Collaborate with community resources and organizations to identify and access housing options for clients.
  4. Conduct thorough assessments to determine eligibility for housing assistance and other support services.
  5. Advocate for clients and assist them in navigating complex housing systems.
  6. Maintain accurate and up-to-date records of client interactions and progress.
  7. Monitor and track client progress towards achieving housing stability goals.
  8. Provide crisis intervention and support to clients in times of need.
  9. Educate clients on housing resources and assist them in understanding and utilizing their options.
  10. Participate in team meetings and contribute to the development of program goals and strategies.
  11. Continuously seek out new resources and partnerships to enhance the services provided to clients.
  12. Maintain confidentiality and adhere to all ethical standards and guidelines.
  13. Stay informed on current trends and best practices in case management and housing services.
  14. Collaborate with medical professionals and other service providers to ensure comprehensive care for clients.
  15. Adhere to all organizational policies and procedures.
  16. Actively participate in professional development opportunities and trainings.
  17. Serve as a positive and supportive member of the team, promoting a collaborative and inclusive work environment.
  18. Comply with all regulatory and funding requirements.
  19. Act as an ambassador for Prisma Health, advocating for our mission and values.
  20. Continuously strive to improve services and outcomes for clients and the community.

Job Qualifications
  • Bachelor's Degree In Social Work, Public Health, Or Related Field

  • Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting

  • Knowledge Of Infectious Diseases And Their Impact On Individuals And Communities

  • Familiarity With Local Housing Resources And Programs For Individuals With Infectious Diseases

  • Excellent Communication And Organizational Skills, With Ability To Work Collaboratively With Interdisciplinary Teams And Community Partners.
