- Educate patients and their families on self-management techniques and resources available to them.
- Maintain accurate and up-to-date documentation of patient care and progress.
- Communicate regularly with patients and their families, as well as with healthcare providers and agencies involved in their care.
- Stay informed about changes and updates in healthcare policies and resources, and apply this knowledge to benefit patients.
- Collaborate with insurance companies and other third-party payers to ensure appropriate coverage for patients' medical needs.
- Participate in interdisciplinary meetings and case conferences to discuss and develop effective treatment plans.
- Adhere to ethical and legal standards in all aspects of patient care.
- Continuously seek opportunities to improve the quality of care and patient outcomes.
- Attend trainings and conferences to expand knowledge and skills related to case management.
Bachelor's Degree In Social Work, Psychology, Counseling, Or A Related Field.
Minimum Of 2 Years Of Experience In Case Management, Preferably In A Healthcare Setting.
Knowledge Of The Healthcare System And Insurance Processes.
Strong Communication And Interpersonal Skills To Effectively Work With Patients, Families, And Healthcare Providers.
Ability To Work Independently And As Part Of A Multidisciplinary Team, With A Focus On Problem-Solving And Advocating For Patient Needs.
