  1. Coordinate and manage the care of patients within the Prisma Health system, ensuring that they receive the best possible care.
  2. Communicate with patients and their families to understand their needs and concerns, and to provide guidance and support throughout their care journey.
  3. Collaborate with healthcare providers, including physicians, nurses, and other care team members, to develop and implement effective care plans for patients.
  4. Utilize knowledge of healthcare regulations and insurance requirements to ensure that patients receive appropriate and timely care.
  5. Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
  6. Maintain accurate and detailed records of patient interactions, care plans, and progress.
  7. Serve as a resource for patients, families, and care team members, providing information and support as needed.
  8. Identify and address potential barriers to care, such as financial constraints or transportation issues, and work to find solutions.
  9. Act as a liaison between patients, providers, and insurance companies to ensure that care is authorized and covered appropriately.
  10. Continuously seek opportunities for process improvement and efficiency in the care management process.
  11. Stay up-to-date on industry trends and best practices to ensure that Prisma Health is providing the highest quality care to patients.
  12. Adhere to all privacy and confidentiality policies and regulations in handling patient information.
  13. Participate in training and professional development opportunities to enhance skills and knowledge.
  14. Represent Prisma Health with professionalism, compassion, and dedication to our mission of providing high-quality care to our patients.

Job Qualifications
  • Bachelor's Degree In Healthcare Management, Nursing, Social Work, Or Related Field.

  • Minimum Of 3 Years Of Experience In Care Management Or Case Management In A Corporate Healthcare Setting.

  • Strong Knowledge Of Healthcare Regulations And Compliance, Including Hipaa And Medicare/Medicaid Guidelines.

  • Exceptional Communication And Interpersonal Skills, With The Ability To Collaborate With Various Healthcare Professionals And Patients.

  • Proficient In Computer Skills And Experience With Electronic Health Records And Care Management Software.
