  1. Provide high-quality pharmaceutical care to patients in an ambulatory setting.
  2. Collaborate with other healthcare professionals to optimize patient outcomes and medication therapy.
  3. Conduct medication reviews and make recommendations to prescribers for appropriate therapy.
  4. Monitor and assess patient response to medications, including potential interactions and adverse effects.
  5. Educate patients on proper medication usage, potential side effects, and medication management.
  6. Maintain accurate and complete patient records, including medication profiles and documentation of interventions.
  7. Stay up-to-date on current pharmaceutical practices, developments, and guidelines.
  8. Participate in medication management initiatives and quality improvement projects.
  9. Communicate effectively with patients, caregivers, and healthcare team members.
  10. Adhere to all regulatory and safety standards.
  11. Contribute to a positive and collaborative work environment.
  12. Serve as a resource for other healthcare professionals regarding medication-related inquiries.
  13. Promote and uphold the mission and values of Prisma Health.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: This Degree Is Required For All Clinical Pharmacist Positions At Prisma Health, And Ensures That The Candidate Has Completed The Necessary Education And Training To Practice As A Pharmacist.

  • Current Pharmacist Licensure In The State Of South Carolina: A Valid And Active Pharmacist License Is A Must For Any Clinical Pharmacist Position At Prisma Health, As It Allows The Candidate To Legally Practice Pharmacy In The State.

  • Minimum Of 2 Years Experience In An Ambulatory Care Setting: This Qualification Is Essential To Demonstrate The Candidate's Familiarity With The Unique Challenges And Responsibilities Of Working In An Ambulatory Care Setting, Where Patients Are Seen On An Outpatient Basis.

  • Board Certification In Ambulatory Care Pharmacy (Bcacp): While Not A Requirement, Having This Certification From The Board Of Pharmacy Specialties (Bps) Is Highly Preferred For An Ambulatory Clinical Pharmacist Position At Prisma Health. It Demonstrates Advanced Knowledge And Expertise In The Field Of Ambulatory Care Pharmacy.

  • Strong Communication And Interpersonal Skills: As An Ambulatory Clinical Pharmacist, The Candidate Will Be Working With A Diverse Group Of Patients, Physicians, And Other Healthcare Professionals. Therefore, Excellent Communication And Interpersonal Skills Are Crucial For Success In This Role.
