
Social Worker - Outpatient Hospice/Palliative

- Provide emotional support and counseling to patients and their families during the end-of-life journey.
- Conduct comprehensive assessments of patients' social and emotional needs and develop individualized care plans.
- Collaborate with interdisciplinary team members to develop and implement a holistic approach to patient care.
- Connect patients and families with necessary resources and community support to improve their quality of life.
- Advocate for patients' rights and ensure their wishes are respected.
- Document all interactions and interventions in a timely and accurate manner.
- Provide education and support to patients and families to help them navigate the hospice/palliative care process.
- Conduct regular follow-up and reassessments to ensure the effectiveness of the care plan.
- Collaborate with community organizations to develop and implement programs and services for hospice/palliative patients.
- Stay updated on current trends and best practices in hospice/palliative care and incorporate them into the role.
- Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
- Participate in team meetings and case conferences to discuss patient progress and address any challenges.
- Maintain accurate and organized patient records and other documentation.
- Adhere to all regulatory standards and laws related to hospice/palliative care.
- Continuously evaluate and improve the social work program to ensure the highest quality of care for patients and families.
Bachelor's Or Master's Degree In Social Work From An Accredited University.
Minimum Of 2 Years Of Experience In A Healthcare Setting, With A Focus On Hospice And/Or Palliative Care.
Knowledge Of State And Federal Regulations Related To Hospice And Palliative Care, As Well As Medicare And Medicaid Guidelines.
Ability To Provide Emotional Support And Counseling To Patients And Families Dealing With End-Of-Life Care.
Strong Communication And Collaboration Skills, With The Ability To Work As Part Of A Multidisciplinary Team.
