RN Care Manager - Presbyterian Heart & Vascular Care
We are seeking an experienced, compassionate, and motivated Registered Nurse (RN) Care Manager to join our Presbyterian Heart & Vascular Care team at Presbyterian Healthcare Services. The RN Care Manager will be responsible for providing patient-centered, evidence-based care and support for patients with heart and vascular conditions. This individual must be knowledgeable and current with nursing practice guidelines to ensure a high quality of care for our patients.The ideal candidate for this position must have a current RN license, along with at least 3 years of experience in cardiac and/or vascular care. We are looking for someone who is able to think critically and assess patient care needs, while maintaining a professional and personable demeanor. The ability to collaborate and communicate effectively with other healthcare team members and departments is essential for this role. -Associate Or Bachelor's Degree In Nursing
