- Serve as a liaison between the outpatient hospice and palliative services and the community, including patients, families, and healthcare professionals.
- Provide compassionate and comprehensive care to hospice and palliative patients, ensuring their physical, emotional, and spiritual needs are met.
- Assess patient needs and collaborate with interdisciplinary team members to develop individualized care plans.
- Advocate for patients by communicating their needs and concerns to appropriate healthcare professionals.
- Educate patients, families, and caregivers on hospice and palliative care services and resources available.
- Facilitate communication and coordination of care between patients and their healthcare providers.
- Conduct regular visits to patients and families to assess their needs and provide support and guidance.
- Document patient progress and maintain accurate and up-to-date medical records.
- Stay current on hospice and palliative care practices, regulations, and policies to ensure compliance and provide high-quality care.
- Participate in interdisciplinary team meetings and collaborate with team members to improve patient outcomes.
- Serve as a resource for staff and community members on hospice and palliative care services.
- Adhere to ethical and professional standards of nursing practice, including maintaining confidentiality and respecting patient rights.
- Participate in quality improvement initiatives to enhance patient care and services.
- Act as an advocate for the organization and promote a positive image in the community.
- Maintain a positive and professional demeanor while interacting with patients, families, and colleagues.
Bachelor's Degree In Nursing: A Registered Nurse In This Role Should Have A Bachelor's Degree In Nursing From An Accredited University Or College. This Ensures A Strong Foundation In Nursing Principles And Prepares The Individual For Advanced Clinical Roles.
Valid Nursing License: A Current And Valid Nursing License Is A Must-Have For A Registered Nurse In This Position. This Ensures That The Individual Is Legally Allowed To Practice Nursing In The State And Can Provide Safe And Quality Care To Patients.
Hospice And Palliative Care Experience: Previous Experience In Hospice And Palliative Care Is Highly Desirable For This Role. It Demonstrates A Strong Understanding Of End-Of-Life Care And The Ability To Provide Compassionate And Holistic Care To Patients And Their Families.
Excellent Communication And Interpersonal Skills: As A Clinical Liaison, A Registered Nurse Should Have Exceptional Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Other Healthcare Professionals. This Includes The Ability To Provide Education And Support To Patients And Their Families During A Difficult Time.
Strong Assessment And Critical Thinking Skills: The Role Of A Clinical Liaison Requires A Registered Nurse To Assess Patients' Needs And Make Critical Decisions Regarding Their Care.
