  1. Provide coordinated and comprehensive care to patients in an inpatient setting.
  2. Collaborate with healthcare team members to develop and implement individualized care plans for patients.
  3. Ensure patient needs are met and their outcomes are improved through proper care coordination and management.
  4. Maintain accurate and up-to-date patient records and documentation.
  5. Monitor and assess patient progress and make necessary adjustments to their care plan.
  6. Serve as a liaison between patients, families, and healthcare providers to facilitate effective communication and promote patient satisfaction.
  7. Utilize strong clinical skills and knowledge to provide safe and high-quality patient care.
  8. Stay up-to-date on current healthcare practices and procedures to continually improve patient care.
  9. Educate patients and their families on disease management, treatment plans, and self-care techniques.

  11. Maintain confidentiality and respect for patients' rights and privacy.
  12. Act as a role model for compassion and empathy in patient care.

Job Qualifications
  • Current Rn Licensure: Applicants Must Hold A Valid And Current Rn License In The State Of New Mexico, Or Be Eligible For Licensure In The State.

  • Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing From An Accredited Institution Is Required For This Position.

  • Minimum Of 3 Years Of Clinical Experience: Candidates Should Have At Least 3 Years Of Experience As An Rn In An Acute Care Setting, With A Strong Background In Care Coordination And Discharge Planning.

  • Knowledge Of Healthcare Regulations And Standards: The Ideal Candidate Will Have A Thorough Understanding Of Medicare And Medicaid Regulations, Joint Commission Standards, And Other Relevant Healthcare Regulations And Standards.

  • Excellent Communication And Interpersonal Skills: This Role Requires Strong Communication And Interpersonal Skills To Effectively Coordinate Care With Patients, Families, And Healthcare Team Members. Candidates Should Also Have The Ability To Advocate For Patients And Collaborate With Other Healthcare Providers.
