Registered Nurse - Hospice & Palliative Care Clinical Liaison

• Develop and maintain relationships with patients and their families to provide compassionate and high-quality hospice and palliative care services
• Serve as a liaison between the hospice and palliative care team and patients’ primary care providers and specialists to ensure continuity of care
• Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs to develop individualized care plans
• Monitor and evaluate patients’ progress and adjust care plans as needed to ensure optimal comfort and quality of life
• Educate patients and their families on end-of-life care options, pain management techniques, and other relevant topics
• Collaborate with interdisciplinary team members, including physicians, social workers, and spiritual counselors, to provide comprehensive and holistic care
• Ensure accurate and timely documentation of patient care and progress in accordance with regulatory standards and company policies
• Provide emotional support and counseling to patients and their families during difficult times
• Participate in quality improvement initiatives to enhance the delivery of hospice and palliative care services
• Stay updated on current trends, best practices, and regulations related to hospice and palliative care to maintain a high level of expertise in the field.
