
Registered Nurse - Hospice & Palliative Care Clinical Liaison

- Develop and maintain relationships with patients and their families to provide compassionate and high-quality hospice and palliative care services
- Serve as a liaison between the hospice and palliative care team and patients’ primary care providers and specialists to ensure continuity of care
- Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs to develop individualized care plans
- Monitor and evaluate patients’ progress and adjust care plans as needed to ensure optimal comfort and quality of life
- Educate patients and their families on end-of-life care options, pain management techniques, and other relevant topics
- Collaborate with interdisciplinary team members, including physicians, social workers, and spiritual counselors, to provide comprehensive and holistic care
- Ensure accurate and timely documentation of patient care and progress in accordance with regulatory standards and company policies
- Provide emotional support and counseling to patients and their families during difficult times
- Participate in quality improvement initiatives to enhance the delivery of hospice and palliative care services
- Stay updated on current trends, best practices, and regulations related to hospice and palliative care to maintain a high level of expertise in the field.
Current Registered Nurse (Rn) License In Good Standing With The State Board Of Nursing.
Minimum Of 2 Years Of Experience In Hospice And Palliative Care Clinical Practice.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Healthcare Team Members.
Knowledge Of Hospice And Palliative Care Regulations And Practices To Ensure Compliance And Quality Of Care.
Ability To Assess And Prioritize Patient Needs, Develop Care Plans, And Coordinate With Interdisciplinary Teams To Provide Comprehensive Care.
