  1. Develop and implement quality programs to improve the health and well-being of communities.
  2. Collaborate with internal and external stakeholders to identify and address gaps in healthcare services.
  3. Lead and support efforts to enhance the quality of care for patients, with a focus on positive health outcomes.
  4. Conduct research and analysis to identify areas for improvement within the healthcare industry.
  5. Monitor and evaluate the effectiveness of quality programs, making recommendations for adjustments and improvements as needed.
  6. Ensure compliance with regulatory standards and requirements related to quality and population health.
  7. Manage and maintain relevant data and metrics to track progress and measure the impact of quality initiatives.
  8. Act as a subject matter expert on population health principles and provide guidance and support to team members.
  9. Develop and deliver training programs to educate staff on quality improvement strategies and best practices.
  10. Stay informed of industry trends and developments related to population health and quality improvement, and make recommendations for incorporating them into our programs.
  11. Foster a culture of continuous quality improvement within the organization.
  12. Serve as a liaison between the Population Health team and other departments within the organization to ensure effective communication and collaboration.
  13. Identify and implement strategies to engage and involve community members in quality improvement efforts.
  14. Represent the organization at meetings, conferences, and other events related to population health and quality improvement.
  15. Uphold the organization's mission, values, and ethical standards in all aspects of the role.

Job Qualifications
  • Strong Understanding Of Population Health Principles And Best Practices: A Quality Program Manager-Population Health Must Possess A Deep Understanding Of Population Health And The Factors That Impact The Health Of A Specific Population. They Should Also Be Knowledgeable About Evidence-Based Practices And Strategies For Improving Population Health Outcomes.

  • Experience Developing And Implementing Quality Improvement Initiatives: This Position Requires Experience In Developing And Implementing Quality Improvement Programs And Initiatives. The Ideal Candidate Should Have A Track Record Of Successfully Leading Projects And Programs That Have Resulted In Improved Health Outcomes.

  • Proficiency In Data Analysis And Interpretation: The Ability To Analyze And Interpret Complex Data Is Essential For A Quality Program Manager-Population Health. They Should Be Able To Use Data To Identify Trends, Gaps, And Opportunities For Improvement, And Make Data-Driven Decisions To Support The Organization's Population Health Goals.

  • Strong Communication And Collaboration Skills: As A Leader In Population Health, The Quality Program Manager Must Have Excellent Communication And Collaboration Skills. They Will Need To Work Closely With Various Stakeholders, Including Healthcare Providers, Community Organizations, And Government Agencies, To Achieve Population Health Goals.

  • Knowledge Of Healthcare Regulations And Compliance: Presbyterian Healthcare Services Operates Within A Highly Regulated Industry, And The Quality Program Manager Must Have A Thorough Understanding Of Healthcare Regulations And Compliance Requirements. This Includes Knowledge Of Quality Reporting Programs, Such As Hedis And Mips, As Well As Understanding Of Hipaa And Other Privacy Laws.
