- Serve as a liaison between the Population Health team and other departments within the organization to ensure effective communication and collaboration.
- Identify and implement strategies to engage and involve community members in quality improvement efforts.
- Represent the organization at meetings, conferences, and other events related to population health and quality improvement.
- Uphold the organization's mission, values, and ethical standards in all aspects of the role.
Strong Understanding Of Population Health Principles And Best Practices: A Quality Program Manager-Population Health Must Possess A Deep Understanding Of Population Health And The Factors That Impact The Health Of A Specific Population. They Should Also Be Knowledgeable About Evidence-Based Practices And Strategies For Improving Population Health Outcomes.
Experience Developing And Implementing Quality Improvement Initiatives: This Position Requires Experience In Developing And Implementing Quality Improvement Programs And Initiatives. The Ideal Candidate Should Have A Track Record Of Successfully Leading Projects And Programs That Have Resulted In Improved Health Outcomes.
Proficiency In Data Analysis And Interpretation: The Ability To Analyze And Interpret Complex Data Is Essential For A Quality Program Manager-Population Health. They Should Be Able To Use Data To Identify Trends, Gaps, And Opportunities For Improvement, And Make Data-Driven Decisions To Support The Organization's Population Health Goals.
