We are currently seeking a compassionate and driven Medical Social Worker Inpatient for our Care Coordination team.   1. Conduct comprehensive psychosocial assessments of patients and their families to identify their needs and concerns.
  2. Collaborate with interdisciplinary teams to develop individualized care plans that address the psychosocial needs of patients.
  3. Provide emotional support and counseling to patients and their families, including crisis intervention as needed.
  4. Connect patients with appropriate community resources and services to meet their ongoing needs after discharge.
  5. Advocate for patients and their families to ensure they receive the best possible care and support during their hospital stay.
  6. Maintain accurate and timely documentation of all patient interactions and interventions.
  7. Participate in interdisciplinary rounds and team meetings to discuss patient progress and develop coordinated care plans.
  8. Collaborate with healthcare providers to address any barriers to discharge and facilitate timely and safe transitions of care.
  9. Provide education and support to patients and their families regarding their healthcare rights and options.
  10. Participate in quality improvement initiatives to enhance the delivery of social work services and patient outcomes.
  11. Keep up-to-date with current social work practices and policies to provide high-quality, evidence-based care to patients.
  12. Foster a positive and collaborative working relationship with other members of the healthcare team.
  13. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  14. Continuously evaluate and improve the effectiveness of social work interventions and services provided.
  15. Contribute to a positive and welcoming atmosphere for patients, families, and staff at Presbyterian Healthcare Services.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Current Licensure As A Medical Social Worker In The State Of New Mexico.

  • Minimum Of 2-3 Years Of Experience In A Healthcare Setting, Preferably In An Inpatient Or Acute Care Environment.

  • Strong Understanding Of Healthcare Systems, Insurance Plans, And Discharge Planning Processes.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Patients From Diverse Backgrounds.
