  1. Conduct patient assessments and provide appropriate treatment plans based on individual needs and medical history.
  2. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients.
  3. Monitor patient progress and make necessary adjustments to treatment plans.
  4. Document all patient interactions and treatment plans accurately and thoroughly.
  5. Communicate effectively with patients and their families to educate them on their health and well-being.
  6. Demonstrate a high level of compassion and empathy towards patients and their families.
  7. Stay up-to-date on current healthcare practices and participate in continuing education opportunities.
  8. Adhere to all medical and ethical standards and maintain patient confidentiality.
  9. Manage in-basket tasks, including reviewing and responding to patient messages, test results, and medication refill requests.
  10. Collaborate with the healthcare team to streamline processes and improve patient care.
  11. Comply with all organizational policies and procedures.
  12. Maintain a positive and respectful attitude towards patients, colleagues, and other healthcare professionals.
  13. Participate in quality improvement initiatives to enhance patient outcomes.
  14. Handle difficult or emergency situations with professionalism and efficiency.
  15. Continuously evaluate and improve own clinical skills and knowledge.
  16. Assist in training and mentoring new team members.
  17. Adhere to all safety protocols and maintain a safe and clean work environment.
  18. Provide excellent customer service and ensure patient satisfaction.
  19. Attend team meetings and actively contribute to discussions and decision-making processes.
  20. Perform any other duties as assigned by the supervisor or management team.

Job Qualifications
  • Master's Degree In Nursing Or Physician Assistant Studies From An Accredited Program.

  • Current And Unrestricted License To Practice As A Nurse Practitioner Or Physician Assistant In The State Of New Mexico.

  • Minimum Of 2 Years Of Clinical Experience In A Primary Care Or Family Medicine Setting.

  • Certification As A Family Nurse Practitioner (Fnp) Or Physician Assistant (Pa) From A Nationally Recognized Organization (Such As Ancc Or Nccpa).

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate Effectively With A Multidisciplinary Healthcare Team.
