
Hospice & Palliative Scheduling Liaison
We are looking for an experienced and passionate Hospice & Palliative Scheduling Liaison to join our team at Presbyterian Healthcare Services. As the Hospice & Palliative Scheduling Liaison, you will be an integral part of our organization, responsible for efficiently and accurately scheduling patient appointments and visits for our hospice and palliative care services. You will work closely with our clinical team to ensure that all of our clients’ needs are met in a timely manner.To be successful in this role, you must have excellent organizational and communication skills, as well as the ability to think on your feet and multitask. You should be customer service-oriented and have a keen attention to detail in order to maintain the highest level of accuracy. We are looking for a self-starter who is comfortable working independently and can make decisions in a fast-paced environment. Responsibilities:
- Schedule patient appointments and visits for hospice and palliative care services in a timely and accurate manner.
- Work collaboratively with the clinical team to ensure that all patient needs are met.
- Maintain a high level of customer service and accuracy when scheduling appointments.
- Respond to patient inquiries and requests in a timely manner.
- Utilize organizational and communication skills to effectively manage multiple tasks.
- Make independent decisions in a fast-paced environment.
- Monitor patient care and provide feedback to the clinical team.
- Remain up-to-date on all relevant policies and procedures.
- Identify and resolve scheduling conflicts.
- Develop and implement strategies to improve scheduling efficiency.
