
Disease Management Health Coach-RN
Presbyterian Healthcare Services is currently seeking an experienced and motivated Disease Management Health Coach-RN to join our team of talented healthcare professionals. The Disease Management Health Coach-RN will be a key member of our team, providing comprehensive care and support to our patients. This individual will be responsible for identifying and assisting patients with chronic illnesses, helping them understand their diagnoses and treatments, and providing education and support to help them manage their conditions.The ideal candidate should possess a minimum of a BSN degree from an accredited school of nursing and current licensure as a Registered Nurse in the state of New Mexico. At least three years of experience in a clinical setting with direct patient care is also required. In addition, the successful candidate should possess excellent communication skills, strong organization and critical thinking abilities, and the ability to work with a variety of professionals. Furthermore, this individual should have a compassionate and caring attitude and the ability to connect to patients and their families. Responsibilities:
- Provide comprehensive medical care and support to patients with chronic illnesses.
- Identify and help patients understand their diagnoses and treatments.
- Educate and support patients to effectively manage their conditions.
- Maintain current licensure as a Registered Nurse in the state of New Mexico.
- Utilize excellent communication skills and critical thinking abilities when interacting with patients and other healthcare professionals.
- Demonstrate a compassionate and caring attitude when connecting with patients and their families.
- Assist in coordinating patient care between medical professionals, social workers, and other healthcare staff.
- Monitor patient progress and provide follow-up care as necessary.
- Develop and implement patient education and support programs.
- Ensure compliance with all applicable laws and regulations.
