  1. Develop and implement educational programs and resources for patients living with diabetes, including group classes, one-on-one counseling, and online resources.
  2. Collaborate with healthcare providers to create individualized treatment plans for patients with diabetes, taking into account their unique needs and preferences.
  3. Conduct thorough assessments of patients' current knowledge and understanding of diabetes, and identify areas for improvement.
  4. Utilize your expertise in endocrinology to educate patients on the disease process, medication management, and lifestyle modifications to effectively manage their condition.
  5. Coordinate with other healthcare professionals, including physicians, nurses, and dietitians, to provide a holistic approach to diabetes management.
  6. Maintain accurate and detailed records of patient interactions, progress, and outcomes in compliance with company policies and regulations.
  7. Stay up-to-date with the latest research and developments in the field of diabetes management and incorporate this knowledge into patient education and treatment plans.
  8. Serve as a resource for patients and their families, answering questions and providing ongoing support and guidance.
  9. Plan and facilitate support groups and community events to promote diabetes education and awareness.
  10. Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction.
  11. Adhere to ethical and professional standards, maintaining confidentiality and respecting patient rights at all times.
  12. Collaborate with other healthcare professionals to identify and address barriers to effective diabetes management, such as access to medication or financial constraints.
  13. Act as an advocate for patients with diabetes, promoting self-management and empowering them to take an active role in their own healthcare.
  14. Attend conferences and seminars to enhance knowledge and skills related to diabetes education and management.
  15. Communicate effectively with patients, their families, and other healthcare professionals to ensure coordinated and comprehensive care.

Job Qualifications
  • Bachelor's Degree In Nursing, Nutrition, Or A Related Field.

  • Certification As A Certified Diabetes Educator (Cde) Through The National Certification Board For Diabetes Educators (Ncbde).

  • Minimum Of 3 Years Of Experience In Endocrinology And Diabetes Management.

  • Knowledge And Understanding Of Current Diabetes Treatment Guidelines And Medications.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Educate And Support Patients With Diabetes.
