Job Description

  1. Coordinate and manage the care of patients in a compassionate and efficient manner.
  2. Collaborate with healthcare providers to create and implement care plans for patients.
  3. Conduct thorough assessments of patients' medical, social, and emotional needs.
  4. Communicate with patients and their families to educate them about their care plan and ensure understanding.
  5. Monitor and evaluate the effectiveness of the care plan and make necessary adjustments.
  6. Ensure timely and appropriate delivery of services and treatments to patients.
  7. Maintain accurate and up-to-date documentation of patient care activities.
  8. Serve as a liaison between patients, healthcare providers, and insurance companies.
  9. Identify and address any barriers to care, such as financial or transportation issues.
  10. Provide emotional support and assistance to patients and families during difficult times.
  11. Stay current on healthcare industry standards and regulations.
  12. Participate in interdisciplinary team meetings to discuss patient care and make recommendations.
  13. Collaborate with community resources to provide comprehensive care for patients.
  14. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
  15. Continuously strive for excellence in patient care and seek opportunities for improvement.

Job Qualifications
  • Strong Communication And Interpersonal Skills

  • Minimum Of 3 Years Experience In Care Coordination Or Case Management

  • Bachelor's Degree In Nursing, Social Work, Or A Related Field

  • Knowledge Of Healthcare Systems, Insurance Plans, And Community Resources
