- Collaborate with healthcare providers to create and implement care plans for patients.
- Conduct thorough assessments of patients' medical, social, and emotional needs.
- Communicate with patients and their families to educate them about their care plan and ensure understanding.
- Monitor and evaluate the effectiveness of the care plan and make necessary adjustments.
- Ensure timely and appropriate delivery of services and treatments to patients.
- Maintain accurate and up-to-date documentation of patient care activities.
- Serve as a liaison between patients, healthcare providers, and insurance companies.
- Identify and address any barriers to care, such as financial or transportation issues.
- Provide emotional support and assistance to patients and families during difficult times.
- Stay current on healthcare industry standards and regulations.
- Participate in interdisciplinary team meetings to discuss patient care and make recommendations.
- Collaborate with community resources to provide comprehensive care for patients.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.

Strong Communication And Interpersonal Skills
Minimum Of 3 Years Experience In Care Coordination Or Case Management
Bachelor's Degree In Nursing, Social Work, Or A Related Field
Knowledge Of Healthcare Systems, Insurance Plans, And Community Resources
Ability To Work Collaboratively With Interdisciplinary Teams And Handle Complex Patient Cases.
