Care Coordinator

Responsibilities:

1. Develop and coordinate individualized care plans to ensure the highest quality of care for patients.
2. Act as a liaison between patients, families, healthcare professionals, and other stakeholders to ensure effective communication.
3. Monitor patient progress and adjust care plans as needed.
4. Facilitate patient referrals to appropriate healthcare professionals.
5. Develop and maintain relationships with community resources to ensure access to necessary services.
6. Provide timely and accurate documentation of patient care plans.

8. Follow all state and federal regulations for the delivery of healthcare services.
9. Collaborate with multidisciplinary teams to provide holistic care to patients.
