
Care Coordinator
Presbyterian Healthcare Services is seeking an experienced Care Coordinator to join our team. Responsibilities:
- Develop and coordinate individualized care plans to ensure the highest quality of care for patients.
- Act as a liaison between patients, families, healthcare professionals, and other stakeholders to ensure effective communication.
- Monitor patient progress and adjust care plans as needed.
- Facilitate patient referrals to appropriate healthcare professionals.
- Develop and maintain relationships with community resources to ensure access to necessary services.
- Provide timely and accurate documentation of patient care plans.
- Maintain a professional and compassionate environment for patients and families.
- Follow all state and federal regulations for the delivery of healthcare services.
- Collaborate with multidisciplinary teams to provide holistic care to patients.
- Participate in professional development and continuing education opportunities.
